QUALIFICATION FOR SURGERY
Qualification for the surgery is the first step to be taken by Patient. A consultation with a doctor is absolutely necessary. The doctor in charge will state recommendation for operative treatment, refer the full diagnostic packet and issue the referral for the surgery.
In order to be scheduled for surgery contracted with National Health Fund a qualifying visit to MedPolonia Outpatient dedicated doctor is necessary.
A visit with a doctor can be arranged in person at Registration Office or on the phone – a pre operative consultation is possible with REFERRAL FOR HOSPITAL issued by GP or a specialist from the proper outpatient clinic.
During a qualifying visit the doctor fills in all documentation needed for Patient to be enrolled on National Health Fund waiting list.
During a qualifying visit Patient is obliged to inform the doctor about all diagnosed chronic diseases and medication taken (also over-the-counter OTC ones!)
